(a) The number of cars to be operated by such person;

(b) The full name of the applicant and the drivers to be in charge of the operation of each car and their places of residence; and

(c) That the applicant has deposited with the City Clerk a policy or certificate of liability insurance in the sum provided in this article for each vehicle for which a license is sought. (Code 1981 § 46-103. Code 1995 § 154-167.)
