(a) The form of the blasting permit shall contain provisions which will hold the City harmless against all costs, expenses, damages and injuries sustained by the City by reason of any operation performed by any person obtaining the permit and operating under the provisions of such permit or by reason of any failure or neglect to comply with any of the provisions or rules and regulations made as set out in this chapter. The permit shall be signed by the applicant therefor agreeing to all conditions contained within the permit.

(b) The blasting permit issued by the City Engineer shall state the location of the proposed blasting, the purpose for which the blasting is being used, the beginning and ending dates covered by the permit and such other pertinent information as the City Engineer may require.
