The license required under this article shall be applied for in writing on forms provided by the City Clerk and shall contain such information as may be required, including, but not limited to, the name of the applicant and taxicab company, and the address of the central place of business. Additionally, the applicant must submit a certified copy of his driving record from the State dated within 30 days of the application for a taxicab driver’s license. (Code 1981 § 46-81.1.
