Applications for dog, cat or miniature pot-belly pig permits shall be made with the Chief of Police or authorized agent upon forms provided by the Chief of Police or authorized agent, setting forth such information as necessary to properly implement the terms and provisions of this chapter. Such forms shall be completed by the applicant and submitted to the Chief of Police or authorized agent for validation and, after validation and payment of the permit fee, such forms shall serve as the permit.
